Self-induced blepharochalasis in a case of facial palsy

Blepharochalasis, is a term used to describe the condition of the skin of upper lid due to thinning and wrinkling, so that the loose fold of skin hangs down over the lid margin. 2 Smaller degree of blepharochalasis may be seen in old age due to loss of elasticity of skin of the lids. In younger age group the pathogenesis is different. It is due to recurrent or chronic odema of the lids leading to stretching of the skin followed by atrophy and relaxation of the skin, this loose skin when the oedema disappears hangs down in front of the upper lid marginal. When blepharochalasis is of larger degree, besides poor cosmetic appearance, it can cause substantial visual disturbance.[1],[3],[4],[5]
Some drooping of the brow and pseudoptosis are seen occasionally as a result of seventh nerve palsy. A case of self induced blepharochalasis is reported here where there was facial palsy. The patient had developed the habit of pulling the skin of the orbit to close the eye, so as to protect it from exposure, and thus produced a pronounced loose fold of skin.

A 60 years Hindu male attended Ophthalmic out patient department for mature cataract in right eye. He was having facial palsy on the same side since last forty years. On examination beside the infranuclear facial palsy on right side, there was pronounced fold of loose orbital skin on outer side which was overhanging the upper lid margin. He was repeatedly pulling the skin to close the eye. The eye was full of water, there was mild degree of paralytic ectorption and chronic conjunctivitis. He was having mature cataract in right eye and immature in left eye.
Before cataract extraction in right eye, the loose redundant fold of skin was excised in first sitting and outer tarsorrhaphy was done in second sitting.

Blepharochalasis is the condition of skin of the upper lid where a loose fold of skin hangs over lid margin due to atrophy and loss of elasticity. Some drooping of the brow and pseudoptosis are seen occasionally in long cases of facial palsy. In this case of facial palsy, to avoid the exposure and irritation, the patient had developed habit of pulling the skin of upper lid and brow on outer side, so as to cover the eye, and produced considerable loose fold of skin.
