Echothiophate Iodide : Its use in accommodative esotropia (High Ac/A Ratio)

Early treatment of accommodative esotro­pia is important if sensory and motor compli­cations are to be avoided. We have used this drug in our ocular motility and amblyopia clinic for near esotropia cases. The effective­ness of echothiophate in reducing near esodevi­ation, its once a day dosage schedule and relative lack of side effects are the main advan­tages.

It is available in three strengths 0.125% 0.0625% and 0.0325%. The dispenser bottles are colour coded for these strengths.


  Materials and methods Top


30 children with near esotopia were divided into A,B,C, groups, and given three types of therapeutic regimen and results were analysed. The following diagnostic criteria was taken in selection of these 30 cases, AC/A ratio more than 4 estimated by hetero phoric method. Refractive error; hyperopia of less than . 1­4.OD. 3. E'T being greater than E.T. by more than 10. Fixation pattern central unsteady central fixation; gross eccentric fixation cases were not considered in this study.

Group A. 10 patients received 0.0625% Ecothiophate Iodide drops once daily along with glasses for distance Group. B. 10 patients were given glasses for distance correction and given + 2.50 D addition for lower segment.

Group C. 10 patients were given glasses for distance correction only.

These three modes of therapeutic regimen were given for four weeks and visual acuity and deviation for distance and near were evaluated weekly.


  Observations and discussion Top


In our series two children complained of burning itching and browache. One child developed epiphora. One child developed pseudo-pemphigoid reaction. The first two cases improved when used along with 10% phenylephrine eye drops. In the third case we discontinued its use. We did not encoun­ter either lens opacities or iris cysts in our series. All cases were notified as "under treatment with ecothiophate iodide" to notify anaesthesiologist.

It is evident from the present study that this drug has certain advantages over the other two regimes. It reduces remarkably the ET and E'T. Once a day drop in each eye is enough. While playing or going on a vaca­tion glasses can be dispensed with. Visual acuity remarkably improved. In Group B. bifocals also remarkably reduced the ET. E'T and visual acuity improved. But the child has to wear glasses constantly. Bifocals adding to cosmetic defect and certain inconvenience to the child. In group C the deviation for distance (E.T) improved but there was little change in near deviation (E T).
