Pharmacokinetics of topically applied sparfloxacin in rabbits

I read with interest the well-written article by Satia et al .[1] regarding pharmacokinetics of topically applied sparfloxacin in rabbits. I need certain clarification from the authors:

  1. The authors mention that the most frequently noted unwanted ocular event associated with ciprofloxacin therapy is a white crystalline precipitate, commonly located in the superficial portion of the corneal area of inflammation and the same is found with ofloxacin therapy. But they did not comment about the crystalline precipitate with the topical use of sparfloxacin.
  2. In relation to topical use of sparfloxacin I would like to point out that prolonged use of sparfloxacin causes refractile crystalline deposits in all layers of the corneal stroma.[2] Deposits can develop in the absence of an epithelial defect, ulceration or inflammation; unlike surface deposits noted with other fluoroquinolones, sparfloxacin is deposited throughout the corneal stroma. The deposits do not cause any inflammation and are asymptomatic. The deposits resolved slowly on discontinuation of sparfloxacin drops.


The authors say that there is a significant increase in the rate of in vitro resistance of Staphylococcus aureus to ciprofloxacin and ofloxacin. Regarding this I would like to say that excessive and inadequate use of any antibiotic whether it may be sparfloxacin develops the resistance to any organisms. No doubt sparfloxacin is a superior quality fluoroquinolone as compared to other fluoroquinolones like ciprofloxacin, lomefloxacin and ofloxacin as an antimicrobial agent and better ocular penetration in aqueous humor and vitreous. Sparfloxacin is effective against resistant microbials also.
