Clinical evaluation of sedyn-a-forte, an analgesic injection containing analgin, diphenhydramine and diazepam

In our preliminary double blind trial a combination of Analgin - 375 mg, Diazepam 2.5 mg and Diphenhydramine 20 mg in post�operative surgical pain and in severe burns, a very good analgesic response was observed. This combination was evaluated in patients with post operative pain after ophthalmic operations for cataracts, glaucoma, keratoplast3 etc., as an analgesic to relieve pain and the analgesic effect was compared with that of 100 mg of pethidine.

 MATERIALS AND METHODS



Injection Sedyn-A-forte, was evaluated in Ophthalmic patients undergoing surgery for cataracts, glaucoma, ulcers, perforation or keratoplasty by open trial.Intensity of pain was graded as 0 - no pain, I - slight pain, 2-moderate pain, and 3-severe pain. Patients were selected for the trial having either moderate or severe pain. They were interviewed before I. M. injection of Sedyn-A-forte was administered and after�wards at an interval of 30 minutes for the first hour and then every 60 minutes for the next 3 hours. Intensity of pain was scored as above. Patients were also asked to describe their pain relief, which was scored as 4 - complete relief, 3 --good relief, 2 moderate relief, 1 - slight relief and 0 -- no relief- Side effects like hangover, nausea, vomiting, dizziness, pain at the site of injection, etc., were also inquired into. The patients acceptance was also graded from excellent to poor and recorded.

The results were analysed by observing pain intensity difference (P. I. D.) for each observation, calculated by substracting the pain intensity at that time from the intensity of pain before the analgesic was administered.

To another group of 21 patients selected at random, I. M. pethidine (100 mg) was administered instead of Inj. Sedyn-A-forte and the relief of pain was recorded as above.

[Table 1], gives the distribution of patients and the mean P. 1. D. values in each group. It can be seen from the table that the drug produced significant relief from pain in post operative cataract cases as well as in the other indications. The pain was abolished in 30 min in almost all the cases, the mean P. I. D. value being 3� 0.02 at 30 min and the pain relief score being 4� 0.03 while in cases of Acute glaucoma or lens induced glaucoma (18 cases), good relief from pain was achieved at 30 min. P. 1. D. value being 2� 0.08 and pain relief score being 3� 0.06 and complete relief from pain was achieved at the end of one hour, (P. 1. D. value 3+ 0.02 and pain relief score 4� 0.03). The P. I. D. and pain relief did not change appreciably at the end of 4 hours. No side effects except giddinses in 6 cases were noted [Table 3]. Patients acceptance was excellent. No second dose was needed after 4 hours.

With pathidine administration similar results were obtained. The mean P. I. D. value being, 3� 0.04 at 30 min and the values pain relief score 4 + 0.04. The P. I. D. pain relief scores remained unchanged at 60 min and upto 4 hours period of observation. The patients on Pethidine administration, however, complained of vomiting (3 cases) and giddiness (3 cases). These side effects were transient and self limiting [Table 3].

Thus, Inj. Sedyn-A-forte seems to equally potent as Inj. Pethidine (100 mg) i its analgesic property and produces nearly th same degree of pain relief when assesse subjectively as above, and is without an appreciable side effect.

 DISCUSSION



Inj. Sedyn-A-forte, contains a centrall acting analgesic Analgin, (375 mg) which' mainly responsible for relief of pain, Diazep (2.5 mg) a centrally acting muscle relaxa and hypnotic sedative, and Diphenhydra�mine, (20 mg) a high-sedative antihistaminic further supplements this effect. The side effects like vomiting as seen with pethidine are also avoided due to addition of diazepam in this combination. Further these drugs have no addiction liabilities and adverse side effects.

This drug can therefore, be used as analge�sic-hypnotic in all cases of post - operative ophthalmic pain and even as a supplement or a replacement to Pethidine in other cases of severe surgical pain.

 SUMMARY



Inj. Sedyn-A-forte, a drug combination containing Analgin, 375 mg, Diazepam 2.5 mg and Diphenhydramine 20 mg per ml. was evaluated for relief of post operative pain in ophthalmic patients and its analgesic effect was compared with that of 100 mg of Pethidine.

A single intramuscular injection of 2 ml of Sedyn-A-forte, produced profound relief from pain at 30 minutes mean P. I. D. being 3� 0.02 and pain relief score 4� 0.03 No hangover or other side effects were noted. Patient's acceptance was excellent. Our results indicate that Sedyn-A-forte is equally potent in relief of pain when compared to Pethidine and thus can be used as an analge�sic in ophthalmic patient and as replacerrent in patients with other severe surgical pain.
