Congenital fistula of the lacrimal sac opening on the skin surface sometimes occurs (Sorsby). More than two puncti are recorded.
But congenital fistula of the lacrimal sac opening on conjunctival surface without any communication with the canaliculi is rather rare.
A Hindu girl K. C. aged about 18 yrs., complained of one opening at the medial side of her right eye ball which she noticed accidentally while examining her eyes in a mirror due to curiosity.
Examination
Rt. eye-The lids and the eye ball are normal. On retracting the upper eye lid upwards one small circular hole 1 mm. in diameter at the supero-medial angle of caruncle is visible. The opening is otherwise invisible. Vision of the eye is 6/6 after myopic correction. Lt. eye; nothing abnormal was detected.
Investigations
Syringing of the right lacrimal passage is done. Both the canaliculi and nasolacrimal duct are patent.
- Lacrimal canula can be passed through the opening of the conjunctival surface without any dilatation and fluid could be easily passed into the nose.
- X' Ray of the lacrimal passage with radio-opaque dye is done. Here the dye (Dionosil) is introduced through the conjunctival opening by a lacrimal canula and film is taken. The presence of dye in the lacrimal sac and nasolacrimal duct is clear from the X' Ray film.

A case of peculiar congenital fistula of the lacrimal sac opening on the conjunctival surface at the supero-medial angle of caruncle is described. The fistula communicating with the lacrimal sac was proved by syringing and X' Ray film after introducing radio opaque dye through the opening.
