Retinal visual acuity in cataractous eyes

Rodenstock retinometer measures retinal and cortical function independent of the influence of the optical media of the eye. It provides quantitative information on the function of the retina rather than the quantitative estimation[1],[2]

In the present study Rodenstock Slit Lamp 2000 model with retinometer was used for this study in which images of two practically punctiform coherent light source were focussed in the comparatively clear area of the lens thus realising Maxwallian principle with a correspondingly high depth of field. Retinometer permits quantitative estimation of visual acuity even in the presence of hazy media.
The validity of the test has been assessed by comparing estimated visual potential with the actual visual acuity acheived after cataract surgery in 60 immature, 40 mature and 10 hypermature cataract cases.

The retinal visual acuity (RVA) Snellen's visual acuity (LVA) and visual acuity with glasses obtained postoperative (LVA) are tabulated in the Table I, II, III, IV. In the present study retinal visual acuity was measured in cataract group with immature, mature and hypermature cases and it was observed that with immature cataract group pre-opera, tive evaluation of visual acuity was better than mature and hypermature cataract when RVA was compared with Snellen's visual acuity it was observed that Retinometer has nearly predicted accurately. It has predicted 6/12 to 6/6 vision in 38.33% cases but gained in 66% cases. Similarly 6/36 to 6/18 was gained 21.33% cases while predicted in 26.33 cases. If we compare retinometer reading in Immature and mature cataract group it will be observed that results of Retinometer in Immature cataract are quite reliable as compared to mature cataract group. Similarly results are more reliable in Immature cataract group.
Thus Retinometer has important value not primarily for patients who ordinarily would be operated on for cataract extraction but in revealing existing potential visual acuit in patients who otherwise would be passed over because of a generally poor prognosis.
Reliability of the retinometer was assessed by comparing estimated visual potential with the actual visual acuity present after cataract surgery. The co-relation between pre-operatively predicted visual acuity and best corrected aphakic vision is high enough to suggest that this test is a useful addition to present methods of preoperatively determining macular function. This is the only method which gives quantitative estimation of visual acuity preoperatively.

Prediction of post operative visual acuity by Retinometer was satisfactory in immature cataraet cases, however, was erratic for eyes with mature and hypermature cataracts.
