Retinal nerve fiber layer measurements by optical coherence tomography in Indian eyes

We read with interest the article on the retinal nerve fiber layer (RNFL) thickness measurements by optical coherence tomography (OCT) in Indian eyes by Ramakrishnan et al .[1] It is an accepted fact that the RNFL measurements show a racial variation. Authors in their article [Table 4] stated a significant variation in the RNFL measurements as reported in various previous studies and these variations may be due the use of different machine models, differences in the scan protocols used and the ethnicity of the population included in the individual studies. The latest version of OCT3 has been provided with an inbuilt normative database for the RNFL measurements; however, it may not be accurate for all patients due to varied differences. [Table 2][Table 3], with the age-wise variation of the RNFL parameters and the percentile distribution of various measured parameters appear to be of great value. Previous studies have shown that the RNFL shows a decrease in the thickness with advancing age. In this study the authors did not observe a significant correlation of the RNFL measurements with advancing age.
The authors mention that prior to their study there were no normative values available for the Indian population. However, we wish to draw their attention to a previous study conducted at the Dr. Rajendra Prasad Centre for Ophthalmic Sciences.[2] This study included 146 normal subjects of Indian origin with 84 males and 62 females. The average RNFL thickness was 104.27 ± 8.51 (95% CI 87.25-121). Age had a significant negative correlation with average RNFL thickness (r = -0.321, P = 0.000) and with average superior (r = -0.233, P = 0.005) and average inferior RNFL thickness (r = -0.234, P = 0.004). There was no effect of gender on various RNFL thickness parameters. This study also provided a guideline for the measurements of the RNFL thickness with OCT in normal Indian eyes. The omission of this study in the present article may be inadvertent, still we feel that this being ours first study in the context of the Indian population, merits a mention and needs to be cited in the present article.
