Ultrastructural study of a contusion cataract

Dear Editor,

We read with great interest the article by Gumus et al .[1] about the electron microscopic findings in a contusion cataract. We have been investigating similar findings in patients with iatrogenic cataracts and their anterior lens capsules. In an ongoing study of four cases, a comparison between the anterior lens epithelieum of one case of traumatic cataract, one of radiation cataract and two of silicon oil-induced cataract, we found histologically more vacuole formation and distortion of the striate layer of the anterior lens capsule. This was not so in the cases with silicon oil-induced secondary cataract, which showed just metaplasia of the epithelieum with very little distortion. We have not found any reference of this fact in the literature. The authors have correctly pointed out that the mitotic activity represents the epithelial response to the disruption in the unicellular arrangement. Hence we feel cataractogenesis is an ultrastructural metaplastic and degenerative alteration of the lens epithelium and capsule and most are regenerative efforts of the lens capsule to the localized insult.

We commend the authors on their observations and feel a larger number of cases should be studied to give more understanding of the delicate balance governing the integrity of the lens epithelium and cataractogenesis.
