Spinning top cataract

A 72-year-old male presented with gradual diminution of vision in the left eye (OS). On oblique illumination, OS had a unique “spinning top” shaped cataract along with cortical cataract [Figure 1]a. Right eye was pseudophakic. Schiempflug imaging [1] showed a zonular lenticular opacity surrounding the fetal nucleus and it was connected to an axial opacity in the anterior capsule through a tract [Figure 1]b. It was successfully removed by phacoemulsification with posterior chamber intraocular lens implantation. Postoperative visual recovery was 20/60. The axial length was 27 mm in both eyes and OS had amblyopia. | Figure 1: (a) Oblique illumination showing “spinning top” shaped cataract.
