A 58-year old male, with a history of smallpox infection in early childhood marked by facial pitting, presented with painless progressive diminution of vision in both eyes [Figure 1]a.[1] Best corrected visual acuity (BCVA) for the right eye was counting fingers close to the face due to mature senile cataract and for the left eye was 20/30. A slit lamp examination of the left eye showed a clear cornea with vitiligo iridis [Figure 1]b.[2] Gonioscopy showed open angles with peripheral anterior synechia [Figure 1]c.[3] Also, it was observed that the intraocular pressures and optic disc were normal. Multiple posterior lenticular opacities were seen and recorded for the first time on pupillary dilatation [Figure 1]d.
