A 13-year-old girl presented with left eye lenticular opacity. The cataractous lens had hexagonal crystals arranged in pyramidal shape [Figure 1] and [Figure 2]. Medical history of abdominal colic and renal stone was obtained. Urine analysis showed small stones and cysteine in urine. Diagnosis of cystinuria with cataract was made. The intralenticular crystals are hexagonal in shape. The hexagonal cystine crystals in urine are similar to that of intralenticular crystals.[1] However, morphological association between cataract and urine crystals has never been documented in literature. Further studies are needed on this line especially at high volume centers with similar patient profile.{Figure 1}{Figure 2}
